How to Register
Welcome to Wells City Practice
We are pleased to accept registration requests from patients who are living within the area indicated on the map below.

Visitors moving to the area
Visitors moving into the area are welcome to register at this surgery.
Please note that we only receive patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area.
If you have family or friends staying who need medical attention, we are able to see them as Temporary patients. Please phone the surgery to make an appointment.
Registering as a patient
You can register at the practice by bringing your medical card to the surgery or signing and completing a registration form which we will supply. If transferring from a local surgery you may be asked to see the GP first before registering.
If you cannot come to the surgery, registration may be carried out by an authorised representative who may sign the registration on your behalf.

Doctors lists
Our list is always open to new patients and we welcome you to register with us.
We do encourage families to use the same doctor where possible. We feel this to be in your best interest.
New patient health check
When you register with the practice you will be invited to make an appointment with our Healthcare Assistant for a new patient health check. This allows any significant health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have.
