Office and practice based within the West Mendip PCN area (Street, Wells or Glastonbury), primarily based in Wells City Practice with occasional home visits and travel to other sites within the PCN area as required.

Experienced Practice Nurse

We are looking for a team player to join our team as a practice nurse with an interest in Diabetes in a patient facing role.

The postholder will work as part of the existing practice nurse team on long-term condition management, complete day to day treatment room tasks, champion public health and prevention, and embed population health principles within general practice.
