What is social prescribing?
Social prescribing is an NHS model that helps connect you with the community to manage your health and wellbeing. There are lots of people, activities, groups and services in the community that can help meet your needs. For example, volunteering roles, cooking classes, sports and more.
Getting a social prescribing link worker
There are local agencies, such as local charities, social care and health services that can refer you to a social prescribing link worker. They can give you support and connect you with what’s going on in the community to help you take control of your mental health and wellbeing.
Mendip Integrated Health & Wellbeing - West Hub
Health Connections Mendip is a health and wellbeing service available to patients. Patients can talk to a health connector at the surgery to find out about local activities, support and services available, and can also arrange one-to-one appointments to speak to a Connector. During these appointments a Health Connector will listen to your health story and assist you with setting health-related goals which are meaningful to you. One-to-one Sessions at Vine Surgery Partnership
Sessions are held at Vine Surgery Partnership on a Wednesday or Thursday each week. If you would like to book an appointment with our Health Connector - Fiona Stobie, please call Health Connections Mendip on 01373 468368. If you would like to have a quick chat with Fiona before booking an appointment, please feel free to ask at reception on those days, and if she's free she'll be happy to talk with you.
Talking Cafés
You can meet new people, make new friends, and chat to a Health Connector about activities, support groups and services in the community.
Street Talking Café takes place at Crispin Community Centre, Street on alternate Wednesdays between 10am and 12pm.
Glastonbury Talking Café is held every Tuesday at Tor Leisure, Street Road, Glastonbury from 10 am to 12.00 pm.

Pain Management Courses
These are for people who experience chronic pain. Friendly, supportive programme of 6 free workshops aiming to equip patients with the self-confidence, knowledge and skills to better manage their health and pain. Courses run at venues across Mendip. Contact the service for details and booking information.
Health Walks
Free local walks are available from all GP practices in West Mendip. Currently there is one walk a month from Vine Surgery Partnership. The aim is for fortnightly walks in the New Year. One-to-one appointments with a Health Connector at Wells Health Centre
Health Connectors support people to make positive changes to improve their health and wellbeing. Health Connectors can listen to your health story and give you information about local services such as exercise classes and support groups. They also work one-to-one to assist people in setting health-related goals and support them to make sustainable changes. This service is free of charge and is open to patients of Mendip GP practices. You can self refer by contacting us on 01373 468368 or emailing mendip.healthconnections@nhs.net.
On Track Group
Come along to these friendly peer led groups where you can join people who are setting goals which might improve their health and wellbeing. Get support and support others, get on track and have a chat over a cup of tea. The On Track groups normally meet every two weeks; please contact us on 01373 468368 to find out specific dates and to register for a place.
